Report Child Abuse Form
If you have reasonable suspicions about child abuse or neglect in cases such:
- Witnessing the Abuse
- Noticing physical or behavioral indicators
- Being confided in by a child or a parent
your immediate action will make a difference!
R Reassure the child that they did the right thing by telling you.
O Overcome the desire to overreact about the abuse or talk negatively about the abuser.
L Listen don’t lead the conversation.
E Encourage the child you are going to help them.
R Recognizing Information: Child’s name, age, and address or their description with
approximate age and where to find the child.
E Eye-witnesses names and contact information
P Parents'/guardians' names and describe their behavior toward/interaction with the child.
O Observations of abuse including type of abuse suspected, when it occurred, and your concern for the child.
R Relationship you are to the child and your knowledge about or suspicion of any past abuse.
T Tell them who done it. Suspected abuser's name and relationship to the child or description of the person and the car license plate number.
A physician, coroner, dentist, medical examiner, nurse, audiologist, certified social worker,
social work technician, school administrator,
school counselor, teacher, law enforcement officer, or duty regulated child care provider
is required by law to report suspected abuse or neglect of a person under 18
years of age by any person responsible for their care.
